​One of the four core principles of Person and Family Centred Care (PFCC) is treating people with dignity and respect. One way to do this is by introducing yourself (name) and your professional role (e.g. nurse, occupational therapist, care aide). Wearing name tags supports this by helping patients, residents and families identify their care team members by name and role.

Over the last six months, the Professional Practice Office and Communications have worked collaboratively to revise the PHC name tags to incorporate the new PHC branding and logo.

If you have submitted a name tag request this year, your new name tag is in production and will be delivered as soon as it is available.​

Name tags provided by PHC will include your preferred name and professional role. You also have the option to add your preferred pronouns.
